Our Personal Supports (PS) Program is designed to work with an individual in his/her home or in the community

Overview

Our Personal Supports (PS) Program is designed to work with an individual in his/her home or in the community. We identify significant lifestyle-related goals with a strategic plan to accomplish these outcomes. Recognizing their specific strengths and weaknesses, individuals receive one-on-one support and assistance from our trained Direct Support Professionals (DSP), including:

  • Skill development centered around daily living, money management, healthy living, emergency preparedness, and recreational engagement.
  • Implementation of learned competencies in the home or community to further address weaknesses, while also developing and working toward lifelong goals and relationships within the community.
  • Access to social, educational, and cultural events and activities.

For individuals within the PS Program, we provide transportation to and from New Horizons as well as to access community activities and, for those independent enough to access public transportation, we provide travel training.
